, on behalf of myself and my child, understand that my child will be participating in the [Trails 4 Tots] trail running program organized by [Prairie Trail Co]. I acknowledge and agree that participation in the Program involves certain risks, including but not limited to risks associated with running, hiking, and other outdoor activities.
In consideration for my child being allowed to participate in the Program, I hereby agree to assume all risks associated with my child's participation in the Program and release [Prairie Trail Co], its officers, directors, employees, and agents from any and all liability arising out of my child's participation in the Program.
I understand that this release of liability applies to any and all claims, demands, or causes of action, whether known or unknown, including but not limited to personal injury, property damage, or wrongful death that may arise out of or be related to my child's participation in the Program.
I further agree to indemnify and hold [Prairie Trail Co], its officers, directors, employees, and agents harmless from any and all claims, demands, or causes of action that may arise out of or be related to my child's participation in the Program.
I have carefully read this Release and Waiver of Liability and fully understand its contents. I understand that this is a binding legal document and I am signing it voluntarily. I further understand that by signing this document, I am giving up certain legal rights that I or my child may have.
As a participant in activities organized by [Prairie Trail Co], I acknowledge and agree to the company's media, video, and photography policy. [Prairie Trail Co] may capture and use media, including photographs and videos, during the course of the activities in which I am participating. I understand that these media may be used for various purposes, including but not limited to marketing, promotional materials, social media, website content, and other company-related communications.
Furthermore, as a parent or legal guardian of a child participating in activities organized by [Prairie Trail Co], I acknowledge and agree that my child's participation may involve being photographed or recorded in videos, and I grant permission for [Prairie Trail Co] to use such media for the purposes mentioned above.
I understand that [Prairie Trail Co] retains all rights to the media captured during the activities, and I waive any claims or demands against [Prairie Trail Co] for the use of such media. I also acknowledge that [Prairie Trail Co] has the right to edit, alter, or modify the media as deemed necessary for its intended purposes.
I further understand that I or my child will not receive any compensation or royalties for the use of the media captured during the activities organized by [Prairie Trail Co]. I release [Prairie Trail Co], its owners, employees, agents, and affiliates from any liability or claims arising from the use of the media captured during the activities.
By signing this participation waiver, I acknowledge that I have read and understood [Prairie Trail Co] participation, media, video, and photography policy, and I voluntarily agree to its terms and conditions and typing parent/guardian name and child’s name act as verifying signatures.
