Please note that failure to provide consent or a positive criminal record check may result in your application being declined or your volunteer placement being terminated.

I understand that false or misleading information may result in my application being declined or my volunteer placement being terminated. I also understand that the personal information provided in this application will be used for the purposes of assessing my suitability as a volunteer with the [Trails 4 Tots] program.
